9. EMERGENCY MEDICAL AUTHORIZATION

In the event of an injury or medical emergency, Maryland Apex Elite Soccer Academy will make reasonable efforts to contact the parent/guardian or emergency contact listed on this registration.

• If I cannot be reached in an emergency, I authorize Maryland Apex Elite Soccer Academy personnel to contact emergency medical services and seek reasonable emergency medical evaluation or treatment for my child. I understand that I am responsible for any medical expenses incurred on behalf of my child.

 

10. PARTICIPATION & ASSUMPTION OF RISK

I understand that participation in soccer, physical exercise, and athletic activities involves inherent risks. These may include, but are not limited to, falls, collisions, strains, sprains, fractures, head injuries, heat-related illness, and other injuries associated with physical activity.

I understand that Maryland Apex Elite Soccer Academy will take reasonable measures to provide a safe training environment but cannot eliminate all risks associated with participation in soccer.

• I have read and acknowledge the participation and assumption-of-risk statement and give permission for my child to participate in Maryland Apex Elite Soccer Academy activities.

 

11. PHOTO & VIDEO PERMISSION

During academy activities, photographs or videos may be taken for academy-related purposes.

12. PARENT CODE OF CONDUCT

Maryland Apex Elite Soccer Academy is committed to providing children with a positive and supportive soccer environment.

As a parent/guardian, I agree to:

• Encourage my child positively.

• Demonstrate good sportsmanship.

• Treat players, coaches, parents, and officials with respect.

• Avoid negative sideline coaching.

• Allow coaches to coach during training.

• Celebrate effort, improvement, and learning.

• Help maintain a safe and welcoming environment for every child.

• I have read and agree to follow the Maryland Apex Elite Soccer Academy Parent Code of Conduct.

 

13. OUR DEVELOPMENT PHILOSOPHY

Maryland Apex Elite Soccer Academy is a development-first soccer academy.

We believe children develop best when they feel safe, supported, encouraged, and free to learn from mistakes.
