Tuition covers the complete 8-week Apex Explorers season and includes:

• 16 one-hour soccer training sessions

• Professional, age-appropriate developmental soccer instruction

• Small-group training with a maximum of 12 players

• All soccer balls and training equipment provided by Maryland Apex Elite

• Official Maryland Apex Elite training jersey

• Apex branded water bottle

• Apex player welcome package

• Players do not need to bring their own soccer ball

Enrollment is limited. Completing this form alone does not reserve a roster spot. Your child's roster spot is confirmed only after the registration form and required tuition payment have both been received.

Parent/Guardian On-Site Requirement

For programs requiring parent/guardian attendance, a parent, legal guardian, or designated responsible adult must remain at the training location for the entire scheduled session.

Maryland Apex Elite Soccer Academy does not provide drop-off childcare or supervision outside scheduled training activities.

PHOTO & VIDEO PERMISSION

Maryland Apex Elite Soccer Academy may occasionally photograph or record academy activities for use on the academy website, official social media accounts, printed promotional materials, and other academy communications.

PARTICIPATION AGREEMENT, ASSUMPTION OF RISK & RELEASE OF LIABILITY

Please Read Carefully

I understand that participation in soccer, soccer training, games, drills, exercises, and other physical activities involves inherent risks.

These risks may include, but are not limited to, running, falling, physical contact with other participants, collisions, contact with soccer balls, goals or other equipment, muscle strains, sprains, fractures, and other injuries.

I voluntarily give permission for my child to participate in activities conducted by Maryland Apex Elite Soccer Academy, LLC and acknowledge and accept the inherent risks associated with participation in soccer and related physical activities.

I certify that I have disclosed any known medical conditions, injuries, allergies, physical limitations, accommodations, or other information relevant to my child's safe participation.

To the fullest extent permitted by applicable law, I, on behalf of myself and my child, agree to release and hold harmless Maryland Apex Elite Soccer Academy, LLC, its owners, coaches, employees, volunteers, agents, and representatives from claims arising from the ordinary and inherent risks associated with participation in academy activities, except to the extent such a release is prohibited or unenforceable under applicable law.

I understand that this agreement is intended to acknowledge and allocate the ordinary risks inherent in participation in youth soccer. Nothing in this agreement is intended to waive any right or claim that cannot legally be waived under applicable Maryland law.

For programs requiring a parent, guardian, or designated responsible adult to remain at the training location, I understand that the responsible adult must remain available during the session in the event the child requires assistance, needs to leave the activity, or experiences an injury or other concern.

By completing this registration, I certify that the information I have provided is accurate and complete to the best of my knowledge and that I am the player's parent/legal guardian or otherwise legally authorized to register the child.
